Medical & Dental Clearance Guide
Follow these simple steps to complete your medical and dental clearance at the Health Services Unit. Prepare your requirements first for a smooth and hassle-free visit.
Before You Visit
Please secure the following Medical Clearance Requirements:

Proceed to the Nurse’s Station
Start your clearance process here.
Clinic Entrance
Nurse’s Station
- Get your queue number from the nurse.
- Fill out your medical record form.
- Proceed to vital signs assessment:
Height
Weight
Temperature
Blood Pressure
Consultation at the Doctor’s Clinic
Review of your health status.

- Proceed to the Doctor’s Clinic as directed by the nurse.
- Have a medical consultation with our doctors.
- The doctor will review your requirements, ask about your health history, and give medical advice or recommendations as needed.
Dental Clearance
Check-up with our dentists.

- Go to the Dental Clinic.
- Undergo a dental check-up and clearance with our dentists:
- The dentist will check your teeth, give advice on oral hygiene, and sign your dental clearance if requirements are met.
Return to the Nurse’s Station
Final steps before you’re done.
- Return your chart and all forms to the nurse for final checking.
- Fill out the short evaluation form about your clinic experience.
- Wait for confirmation that your Medical and Dental Clearance is complete.

– Medical and dental clearance for enrollment, OJT, and events. -
First Aid & Emergency Care
– Immediate response for accidents and urgent cases. -
Dispensing of Medicines
– Free or prescribed medicines for minor illnesses. -
Health Talks & Seminars
– Educational programs to promote healthy lifestyles.

Step 1: Arrival and Registration
- Check in at the reception desk.
- Provide your personal details and student/employee ID if applicable.
- Wait for your turn in the designated waiting area.
Step 2: Medical Consultation
- Meet with the healthcare professional assigned to your case.
- Discuss symptoms or health concerns openly.
- Follow the instructions provided, including tests or procedures if necessary.
Step 3: Treatment or Referral
- Receive treatment on-site if possible.
- Get prescriptions or referrals for specialized care if needed.
- Follow-up appointments may be scheduled if required.
